  • Covered California health plans include dental for children under 19.

  • Waiting periods differ a lot by carrier, so compare them before you buy.

How Does Dental and Vision Insurance Work in California?

Most Californians get dental and vision help from one of three places. First, a private policy you buy directly. Second, Covered California. Third, Medi-Cal, if your income qualifies. Private plans are the most flexible. For instance, you can apply any month, and you don't need a health plan with the same company. That's why many people pair one with an ACA plan, a work plan without dental or Medicare. Covered California works differently. For example, every health plan there includes core dental care for children under 19 at no extra cost. Adults, however, can buy a family dental plan only as an add-on to a Covered California health plan. Medi-Cal also includes dental benefits through Medi-Cal Dental. However, Smile California lists benefit changes for some adult members starting July 1, 2027. If you have Medi-Cal, check with Medi-Cal Dental before you buy a second plan. Want the bigger picture? Covered California Dental or a Private Plan?

Both can work well, so the right pick depends on your situation. Here are the main differences to weigh.
QuestionCovered California family dentalPrivate dental and vision plan
Need a health plan first?Yes, a Covered California health planNo
When can you apply? When you enroll in your health planAny month of the year
Vision included? No, dental onlyOptional, through bundles or separate plans
Children under 19Core dental already built into the health planCan join a family policy
For example, an adult with a work health plan can't use Covered California dental. In that case, a private plan is the main route. Similarly, a family on Covered California may skip a child's dental plan, yet still want vision coverage for everyone. Meanwhile, some people want dental care this month. Because private plans accept applications year-round, they can start sooner. As a result, many shoppers compare both before they decide. Not sure which way to go? What Do UnitedHealthcare Plans Pay in California?

UnitedHealthcare sells California versions of its plans, underwritten by Golden Rule Insurance Company. Overall, the benefits match the plans sold in most other states.
PlanYearly maximumMajor careImplantsVision and hearing
DentalWise Basic$1,000Not coveredNot coveredNo
DentalWise Plan 1000$1,00015% year one, then 50%Not coveredNo
DentalWise Plan 2000$2,00015% year one, then 50%50% after 12 monthsNo
DentalWise Max Plan 1000$1,00015% year one, then 50%Not coveredYes
DentalWise Max Plan 2000$2,00015% year one, then 50%50% after 12 monthsYes
DentalWise Max Plan 3000$3,00015% year one, then 50%50% after 12 monthsYes
Every plan pays 100% for preventive care, with no deductible. Basic care, like simple fillings, pays 60% in the first policy year and 80% after that. Each person has a $100 deductible per policy year. In addition, implant benefits carry a $1,500 lifetime maximum. That limit is separate from the yearly maximum. Meanwhile, no DentalWise plan covers braces, so families who need orthodontia should compare Cigna too. Want the full plan details? How Do Vision Benefits Compare in California?

You can buy vision coverage alone or inside a bundle. First, here's how the main choices line up for network care.
PlanEye examFramesContacts
UnitedHealthcare VisionWise$0Up to $150Up to $150
DentalWise Max vision benefit$0Up to $150Up to $150, after a $10 copay
Aetna Vision Preferred Direct Value$20 copay$130$130
Aetna Vision Preferred Direct Select$15 copay$160$160
Aetna Vision Preferred Direct Elite$10 copay$200$200
VisionWise lenses cost a $10 copay in the network. However, out of network, it pays set allowances, such as $50 toward an exam. Aetna's vision plans also include out-of-network coverage. Cigna sells vision too, but mostly inside its dental bundles. So, if you want one bill for teeth and eyes, compare DentalWise Max with a Cigna bundle. Our United Healthcare vision insurance guide explains VisionWise in more depth.

Which Plans Start Right Away?

Waiting periods are where these plans differ the most. So, before you buy, match the plan to the care you need soon. Our guide to dental insurance with no waiting period explains the trade-offs.
  • UnitedHealthcare: no waiting period for preventive, basic or major care. Only implants wait 12 months.
  • Aetna: preventive care starts right away. Waits for basic and major care are waived if you had dental coverage within 90 days.
  • Cigna: basic care waits 6 months and major care waits 12 months. Both waits can be waived with qualifying prior coverage.
  • Aflac: basic care has no wait on the Advantage and Premier plans. Major care waits 6 months, with credit for recent prior coverage.
In contrast, vision plans usually start fast. VisionWise and Aetna's vision plans have no waiting period at all. There's a trade-off, though. UnitedHealthcare pays only 15% for major care in year one. Therefore, a big repair may cost less on a plan with a waiting period, once that wait ends. Here's a simple example, with made-up numbers. Say a crown costs $1,200 at a network dentist. In year one, a 15% benefit pays $180, before the deductible. By contrast, a plan paying 50% after its wait would pay $600. So, timing matters as much as the plan name. If the crown can wait a year, the waiting-period plan may come out ahead. But when it can't wait, the day-one plan still helps.

How Do You Find a Dentist or Eye Doctor in California?

Each carrier uses its own network. So, check yours before you apply, because staying in the network usually lowers your cost.
  • UnitedHealthcare dental: National Options PPO 30, searchable at yourdentalplan.com/dentistsearch.
  • Aetna dental: the Aetna Dental PPO network, with over 600,000 dentist locations.
  • UnitedHealthcare vision: the UnitedHealthcare Vision Network, at myuhcvision.com.
  • Aetna vision: the EyeMed network, including LensCrafters, Pearle Vision and Target Optical.
Next, call the office and ask about your exact plan name. That quick call can save you a surprise bill later. Our guides on dentists that accept United Healthcare and how to find an in-network dentist walk through each step.

Who Should Buy a Private Dental or Vision Plan?

In general, a private plan tends to fit Californians who fall between the public programs and a work plan.
  • Self-employed workers: people with no dental or vision benefits through a job.
  • Adults without Covered California: people who can't add a family dental plan there.
  • Medicare members: Original Medicare doesn't cover most routine dental or vision care.
  • Couples and domestic partners: UnitedHealthcare's California plans cover registered domestic partners.
On the other hand, Medi-Cal members may already have dental benefits. Likewise, children on a Covered California plan already have core dental care. In those cases, a vision-only plan may be all you need.

Dental and Vision Insurance California FAQs

Can I buy dental insurance in California without a health plan?

Yes. In fact, private plans from UnitedHealthcare, Cigna, Aetna and Aflac don't require a health plan. You can apply any time of year. Covered California is the exception. There, adults must have a Covered California health plan before adding family dental.

Do my kids need a separate dental plan in California?

Not always. That's because Covered California health plans include core dental care for children under 19 at no extra cost. Still, your health plan picks the dental company for your child. If you want a different one, you can buy a separate family dental plan.

Can I cover my domestic partner?

Yes, on UnitedHealthcare's California plans. They cover a spouse or registered domestic partner, as defined in California Family Code section 297. Children under 26 can join too. Call 888-730-6031 to check other carriers' rules.
