Do insurance deductibles start over? Yes. Most health insurance deductibles start over once a year. For ACA Marketplace and most individual plans, the deductible resets on January 1, because those plans run on the calendar year. Job-based plans reset at the start of their plan year, which may be a different month. Your out-of-pocket maximum resets at the same time. Switching plans usually starts your deductible over at $0 as well, so timing your care can save you real money.

Key takeaways
  • Marketplace and most individual plans reset deductibles every January 1.
  • Employer plans reset on the first day of the plan year, which you'll find in your plan documents.
  • Changing plans mid-year usually restarts your deductible at $0.
  • For 2027, the out-of-pocket maximum rises to $12,000 for one person and $24,000 for a family.

What Else Resets With Your Deductible

Your deductible isn't the only thing that starts over. On the same date, these usually reset too:

  • Out-of-pocket maximum. Your yearly spending cap goes back to $0 progress.
  • Visit and benefit limits. For example, a limit of 20 therapy visits a year starts fresh.
  • Prescription deductibles. Some plans have a separate drug deductible that resets as well.

Meanwhile, your premium may change on January 1, too. See why health insurance premiums are going up for what to expect.

Licensed agent tip

Already met your deductible this year? Then book any planned care before December 31. For example, a scan or procedure in December may cost far less than the same one in January.

Switching Plans and Your Deductible

When you change plans, your new plan doesn't usually count what you paid on the old one. So your deductible starts over at $0. That applies whether you switch during Open Enrollment or after a life event.

Because Marketplace plans all reset January 1 anyway, switching during Open Enrollment costs you nothing extra. In contrast, switching mid-year after a move or job change can mean paying a second deductible. Read how switching health plans works before you decide, or call 888-730-6001 to talk it through.

If your insurer is leaving the Marketplace, the same rule applies. Your new 2027 plan starts at $0 on January 1. Learn more in our guide to an insurance company leaving the Marketplace.

How Family Deductibles Reset

Family plans have two layers. First, each person has an individual deductible. Second, the family has a combined deductible.

  • Embedded deductible. Once one person meets their own deductible, the plan starts paying for that person.
  • Aggregate deductible. Instead, the whole family must meet one combined amount before the plan pays.

Either way, both amounts reset on the same date. In addition, no one person pays more than the individual out-of-pocket maximum on an ACA plan.

How to Time Your Care

  1. Check your progress. Log in to your insurer's site to see how much you've paid toward this year's deductible.
  2. Book planned care early. If you're close to your deductible, schedule care before it resets.
  3. Refill prescriptions. Ask about a 90-day supply before December 31 if your plan allows it.
  4. Use free preventive care. Checkups and many screenings cost $0 any time of year.
  5. Pick next year's plan. Then compare deductibles during Open Enrollment, by December 15 for January 1 coverage.
